Value Description Select a permissible use from the drop-down list. Strict Search Select this if you only want to search for the exact spelling of the names you enter. If you select Strict Search , you cannot select Find similar-sounding last names or Find nicknames of first name if they appear on the form (clear this box if you want to select one of those checkboxes). Enter the last name of the person you are searching for. If you are not sure of the correct spelling, select the box next to “Use similar-sounding terms for Last Name.” Your search will find names that match, sound like, or are spelled similarly to the name.
For example, if you enter "klein", your search will also find records with the last names "Cline" and "Kline".
Enter the first name or initial of the person you are searching for. Enter the middle name or initial of the person you are searching for. SSN You can use a Social Security Number as a search term. However, if you do not possess the proper authorization, such as a member of a recognized law enforcement agency, it will not appear in any of the retrieved information.

Date of Birth You can use a birth date to narrow your search. Searching on date of birth alone will not produce any results; you must enter a last name or a street address in addition to the birth date.

Enter the city of residence for the person you are looking for. Your search will find results for the exact city name as well as for associated city names.
For example, if you enter skokie (and Illinois for the state) Your search will find records for Chicago as well as for Skokie.
You can enter a zip code to limit your search to a specific geographical area. We recommend you limit your entry to the 5-digit form most commonly used. Enter the name of the county the person resides in. Address Type Choose an address type or "All Address Types" from the drop-down list. The Person Report search form will search within a radius of the location you specify. The radius is based on the zip code, if you enter one. If not, the radius is based on the city/state combination you enter. Select a radius from the drop-down list. Select the source type or select "All Available Source Types" from the drop-down list. Click the "Search" button to begin your search. Click the "Clear Form" link to clear all the data you have entered in the fields.
