Use this page to indicate how you would like to receive your One-Time Passcode (OTP).

Value	Description
Choose how you want to receive your passcode	Use these radio buttons to select how would like to receive your OTP. Note: If you select one of the Telephone radio buttons, you must also select whether you want to receive the OTP via text or voice message.
Send OTP	Select to have a unique OTP sent in the selected format.
Cancel	Select to return to the last page you were viewing without taking any action.
